• First Responder Authentication Credential Identification Card Application
  Please fill out every field on application and have the Section Chief, Branch Director or emergency coordination officer (ECO) of your Emergency Support Function (ESF) sign off on the form prior to appointments. If you have any questions about who the ECO is for your ESF please contact the State Watch Office at 850-413-9900. Once complete, please e-mail the application to the FDEM Security Office at security@em.myflorida.com. Once approved, you may make an appointment by e-mailing Mindy Dowling @ mindy.dowling@em.myflorida.com.
